Each box here is one large box of ECG paper (0.2 seconds) and each heavy line marks a full second; the small 0.04-second boxes are not drawn, and on real paper the heavy lines are the large boxes. So 300 divided by the boxes between two R waves works on screen, and so does counting the complexes and multiplying by ten; 1500 divided by small boxes does not, and PR and QRS widths are estimates against one box. A simulated single lead is for learning the patterns - on the unit, read the patient and the full monitor.
